초록/요약
내시경의 발전에 따라 최소 절제를 통해 이루어지는 최소 침습 수술과 이를 활용한 로봇 수술에 대한 연구가 활발히 진행되고 있다. 대표적인 예시로, 전립선 수술은 복강 및 골반 내에 깊숙이 위치한 해부학적 특성으로 인해 고난도의 기술이 요구된다. 또한 기존의 형광 유도 암 절제술은 높은 국소 재발률과 심부암 탐색의 한계라는 단점을 가지고 있다. 이러한 한계점을 극복하고자, 본 연구에서는 핵의학/형광 융합 영상 기반 정밀 암 치료용 테라노스틱 플랫폼의 핵심 요소로 초소형 감마선 검출 프로브를 설계하였다. 개발된 프로브 및 로봇 기관지경을 이용한다면, 심부암 탐색이 어려웠던 기존 치료법의 한계를 극복하고, 국소/전이암에 효과적인 치료법인 PDT(Photo Dynamic Therapy)/PTT(Photo Thermal Therapy) 융합 치료법을 적용할 수 있게 된다. 현재 상용중인 Intuitive 사의 da Vinci ION로봇 기관지경에 이를 적용하기 위해서는 전체 직경 3.5 mm, working channel 직경 2 mm라는 조건을 만족시켜야 한다. 그러나 이와 같은 공간적 제약에 의해 후방에collimator를 장착할 수 없기 때문에, 수술 중 측면과 후면으로부터 입사하는 배경 방사선을 차폐할 수 없다. 특히 본 프로브는 체내에서 진행되는 암 수술에 활용되는 만큼, 인체 내부에 존재하는 배경 방사선을 차단하는 것은 정밀한 병변 탐색과 오탐 방지를 위해 필수적인 요소이다. 따라서 이를 해결하기 위해 본 연구에서는 기존의 lightguide 대신 BGO 섬광체를 사용한 phoswich 구조의 초소형 감마선 검출 프로브를 설계하였다. 후방에서 입사하는 배경 방사선은 BGO 섬광체와 우선적으로 반응하게 되고, 이를 통해 형성된 신호의 파형을 기반으로 파형 분별법(Pulse Shape Discrimination, PSD)을 적용함으로써GAGG 섬광체, BGO 섬광체 각각과 반응하여 서로 다르게 성형된 목표 방사선 신호와 배경 방사선 신호를 효과적으로 구분할 수 있도록 하였다. 특히 기존에는 raw 신호에만 적용이 가능했던 PSD 기법을, 본 연구에서는 전치 증폭기(pre-amplifier)의 일종인 charge sensitive pre-amplifier(CSP)를 거친 신호에도 PSD를 적용할 수 있도록 구현하여 적용한 점이 본 연구의 주요 특징이다. 결과적으로, 이와 같은 방법을 통해 측면 및 후면에서 입사하는 감마선의 약 85%를 BGO 섬광체와 반응시킬 수 있었으며, PSD 기법을 통해 분리 성능 지표(Figure of Merit, FOM) 1.8의 우수한 성능을 보이며 배경 방사선을 분리 및 제거할 수 있었다. 본 연구에서 개발된 초소형 감마선 검출 프로브는 로봇 기관지경 시스템에 통합되어 실시간 정밀 암 탐색을 가능하게 함으로써, 향후 핵의학/형광 융합 영상 유도 및PDT(Photo Dynamic Therapy), PTT(Photo Thermal Therapy) 등 정밀 치료 전략의 기반 기술이 될 것으로 기대된다.

With the advancement of endoscopy, research on Minimally Invasive Surgery involving minimal resection and its application in robotic surgery is actively progressing. As a representative example, prostate surgery demands highly sophisticated techniques due to the deep anatomical location within the abdominal and pelvic cavities. Furthermore, conventional fluorescence-guided cancer resection suffers from drawbacks such as high local recurrence rates and limitations in deep-seated tumor detection. To overcome these limitations, this study designed a miniature gamma detection probe as a key component of a theranostic platform for nuclear/fluorescence fusion imaging-based precision cancer therapy. Utilizing the developed probe with robotic bronchoscopy could overcome the limitations of conventional methods in detecting deep-seated tumors, potentially enabling the application of combined PDT (Photodynamic therapy)/PTT (Photothermal therapy), effective treatments for local/metastatic cancers. For its application in the commercially available da Vinci ION robotic bronchoscope system by Intuitive Surgical, the probe must meet the dimensional constraints of an overall diameter of 3.5 mm and fit within a 2 mm working channel. However, these spatial constraints prevent the attachment of a rear collimator, making it impossible to shield background radiation incident from the sides and rear during surgery. As this probe is intended for intraoperative use in cancer surgery, blocking internal background radiation is crucial for precise lesion detection and preventing false positives. Therefore, to address this issue, we designed a miniature gamma probe with a phoswich structure using a BGO scintillator instead of a conventional lightguide. Background radiation incident from the rear preferentially interacts with the BGO scintillator. Based on the waveform of the resulting signal, Pulse Shape Discrimination (PSD) was applied to effectively distinguish between target radiation signals (from interactions with the GAGG scintillator) and background radiation signals (from interactions with the BGO scintillator), which have distinct pulse shapes. A key technical feature of this study is implementation and application of the PSD technique, conventionally applicable only to raw signals, to signals processed by a pre-amplifier, specifically a charge sensitive pre-amplifier (CSP). Consequently, this approach allowed approximately 85% of gamma rays incident from the side and rear to interact with the BGO scintillator, and the PSD technique demonstrated excellent performance in separating and rejecting background radiation, achieving a Figure of Merit (FOM) of 1.8. The miniature gamma detection probe developed in this study, integrated into robotic bronchoscopy systems, enables real-time precision cancer detection. It is expected to serve as a foundational technology for future precision treatment strategies, such as nuclear/fluorescence fusion image guidance and therapies like PDT/PTT.
