복부비만 성인남성의 운동강도별 뇌혈류 속도 및 혈관 저항지수 변화

초록/요약

본 연구의 목적은 운동부하 검사 시 운동강도에 (저강도 운동 HRR 35%, 중강도 운동 HRR 50%, 고강도 운동 HRR 80%) 따른 복부비만 성인남성의 뇌혈류 속도 변화, 혈관 저항지수 및 혈압 차이를 비교 분석하여 운동 중에 발생될 수 있는 뇌혈관 질환 사고를 예방하고 최적의 운동강도를 규명하여 건강을 증진시키는데 기여하고자 실시되었다. 연구 대상자는 20 대 ~ 40 대 성인 남성으로 허리둘레 90 cm 이상인 36 명을 대상으로 하였으며, 사전검사에서 부정맥 1 명, 불량 탐촉창 2 명, 중증도 이상 협착소견 1 명 등 4 명이 탈락하게 되어 총 32명을 대상으로 연구를 진행하였다. 프로그램은 Bruce protocol을 적용한 TMT를 실시하였으며 운동강도 변화에 따른 뇌혈류 속도, 혈관 저항지수 및 혈압을 실시간으로 관찰하였다. 수집된 모든 자료는 Windows 용 IBM SPSS 27 통계프로그램을 이용하여 평균 및 표준오차를 산출하였으며, 운동강도에 따른 뇌혈류 속도, 혈관 저항지수 및 혈압 변화의 통계처리는 one-way repeated measures ANOVA 를 실시하고 유의한 차이가 있을 경우 Bonferroni 사후분석을 실시하였다. 정규성이 만족되지 않은 혈관 저항지수는 비모수 검증ii Friedman test 실시하였으며, 모든 통계적 유의수준은 P<0.05 로 설정하였다. 본 연구의 결과는 다음과 같다. 1. 운동강도 변화에 따른 뇌혈류 속도는 모든 강도에서 유의한 차이를 보여주었으며 (P<.001), 안정시와 고강도 운동간 차이가 가장 크게 나타났다. 2. 운동강도 변화에 따른 혈관 저항지수는 박동지수와 저항지수 모두 유의한 차이를 보여주었으며 (P<.001), 구간별 비교에서 중강도 운동과 고강도 운동에서 박동지수 (P=.46)와 저항지수 (P=.36) 모두 유의한 차이가 없었다. 3. 운동강도 변화에 따른 혈압은 수축기 혈압과 이완기 혈압 모두 유의한 차이를 보여주었다 (P<.001). 특히 수축기 혈압은 모든 구간에서 유의한 차이를 보였으나 (P<.001), 이완기 혈압은 안정시와 저강도 운동, 중강도 운동, 고강도 운동에서 유의한 차이를 보였으며 (P<.001), 저강도 운동과 중강도 운동, 저강도 운동과 고강도 운동, 중강도 운동과 고강도 운동 구간에서는 혈압 차이는 있었으나 유의한 차이는 없었다 (P=1.00). 운동강도별 뇌혈류 속도 변화에 관한 연구는 운동형태와 운동강도 설정방법 및 뇌혈류 속도 측정방법에 따라 다양하게 연구되어왔다. 본 연구는 운동 중 실시간으로 뇌혈류 속도를 관찰하였으며 운동강도가 증가함에 따라 뇌혈류 속도, 혈관 저항지수, 혈압이 증가되었다. 특히 새로운 뇌혈류 속도 측정방법을 제시하면서 측정 오류에서 비롯된 결과 정확도를 향상시키고 나아가 무리한 운동으로 발생되는 비정상적 뇌혈류와 혈압 변화를 예측하여 개인별 적정 강도를 고려한 운동 처방을 제시하는데 이바지하고자 한다.

The purpose of this study was to compare and analyze changes in cerebral blood flow velocity, vascular resistance index, and blood pressure in adult males with abdominal obesity according to exercise intensity during exercise stress testing (low-intensity exercise at HRR 35%, moderate-intensity exercise at HRR 50%, and high-intensity exercise at HRR 80%). The aim was to contribute to health promotion by preventing cerebrovascular disease incidents that may occur during exercise and determining optimal exercise intensity. The study involved 36 adult males aged in their 20s to 40s with a waist circumference of 90 cm or more. However, after a baseline assessment, four participants were excluded due to arrhythmia (1 participant), poor temporal window (2 participants), and moderate or severe stenosis (1 participant), resulting in a final study group of 32 participants. The program used the Bruce protocol-based treadmill test (TMT) to observe changes in cerebral blood flow velocity, vascular resistance index, and blood pressure in real time based on varying exercise intensities. All collected data were processed using the IBM SPSS 27 statistical program for Windows to calculate mean values and standard errors. The statistical processing for changes in cerebral blood flow velocity, vascular resistance index, and blood pressure according to exercise intensity was performed using one-way repeated measures ANOVA. If a significant difference was observed, Bonferroni post-hoc analysis was conducted. The vascular resistance index, which did not satisfy normality, was analyzed using the non-parametric Friedman test, and all statistical significance levels were set at P<0.05. The results of this study were as follows: 1. Cerebral blood flow velocity showed a significant difference across all exercise intensities (P<.001), with the largest difference observed between resting and highintensity exercise. 2. The vascular resistance index, including both the pulsatility index and resistance index, demonstrated significant differences across exercise intensities (P<.001). In interval comparisons, there was no significant difference between moderateintensity and high-intensity exercise for both the pulsatility index (P=0.46) and the resistance index (P=0.36). 3. Blood pressure showed significant differences for both systolic and diastolic blood pressures according to exercise intensity (P<.001). Systolic blood pressure exhibited significant differences across all intervals (P<.001), while diastolic blood pressure showed significant differences between rest and low-intensity exercise, moderateintensity exercise, and high-intensity exercise (P<.001). However, there were differences in blood pressure between low and moderate, low and high, and moderate and high-intensity exercise intervals, but these were not statistically significant (P=1.00). Research on changes in cerebral blood flow velocity according to exercise intensity has been conducted in various ways depending on the type of exercise, exercise intensity settings, and measurement methods for cerebral blood flow velocity. This study observed cerebral blood flow velocity in real time during exercise, demonstrating that as exercise intensity increased, cerebral blood flow velocity, vascular resistance index, and blood pressure also increased. By presenting a new measurement method for cerebral blood flow velocity, this study aimed to improve result accuracy derived from measurement errors and contribute to predicting abnormal changes in cerebral blood flow and blood pressure caused by excessive exercise, thereby proposing exercise prescriptions tailored to individual optimal intensities.
