Correlation between Hearing Thresholds and Chronic Rhinosinusitis : Analysis of the Korea National Health and Nutrition Examination Survey (KNHANES) 2009-2012

Aim: This study aimed to investigate the relationship between chronic rhinosinusitis (CRS) and hearing thresholds assessed through pure tone audiometry (PTA) using data from the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: In this cross-sectional study, data from participants of the KNHANES collected between 2008 and 2012 were analyzed. CRS was diagnosed based on questionnaire responses and nasal endoscopy findings, while PTA and otologic examinations were performed by otolaryngologists. To evaluate the association between chronic rhinosinusitis and hearing thresholds, multivariate logistic regression analysis was conducted after adjusting for potential confounding variables. Results: Participants with CRS exhibited significantly higher hearing thresholds, including 500Hz, 1000Hz, 2000Hz, 3000Hz, 4000Hz, low tones average, mid-tones average, high tones average, and average of 0.5k, 1k, 2k, and 4kHz in both right and left ears (p < 0.05). After adjusting for confounding factors, worse ear average hearing threshold was found to be significantly associated with CRS (adjusted OR 1.053, 95% CI 1.005-1.104, p=0.0304). Furthermore, worse ear low tones average hearing threshold was significantly associated with CRS (adjusted OR 1.052, 95% CI 1.004-1.104, p=0.0350). For every 10dB increase in hearing thresholds, there was a significant decrease in the odds of reduction of smell (adjusted OR 0.847, 95% CI 0.755-0.951, p=0.0050). Conclusion: This study demonstrates a significant association between CRS and elevated hearing thresholds, particularly in the worse-hearing ear. Olfactory dysfunction is correlated with reduced hearing thresholds in CRS patients. These findings underscore the importance of considering hearing assessment in CRS management, potentially improving the quality of life for affected individuals. Further research is needed to explore the underlying mechanisms of this association and its clinical implications.

목적: 이 연구는 국민건강영양조사 자료를 바탕으로 하여 만성 부비동염과 순음청력검사를 통해 평가된 청력 역치 간의 상관 관계를 분석하고자 한 논문이다. 방법: 이 단면적 연구는 2009년부터 2012년까지 국민건강영양조사에 참여한 만 18세 이상의 성인 11,210명의 데이터를 분석했다. 만성 부비동염은 설문지와 내시경 소견에 근거하여 진단되었으며, 순음청력검사와 이과적 진찰은 이비인후과 전문의에 의해 시행되었다. 만성 부비동염과 청력 역치 간의 상관 관계를 평가하기 위해 잠재적 교란 변수를 보정한 후 다변량 로지스틱 회귀 분석을 시행하였다. 결과: 만성 부비동염을 가진 참가자들은 500Hz, 1000Hz, 2000Hz, 3000Hz, 4000Hz, 저음 평균, 중음 평균, 고음 평균 및 0.5kHz, 1kHz, 2kHz, 4kHz의 평균 청력 역치가 양쪽 귀에서 유의미하게 높게 나타났다. 교란 변수를 보정한 후에도 청력이 더 나쁜 귀의 청력 역치가 만성 부비동염과 유의미한 상관 관계가 있는 것으로 나타났다(보정된 교차비 1.053, 95% 신뢰구간 1.005-1.104). 또한, 청력이 더 나쁜 귀의 저음 평균 청력 역치가 만성 부비동염과 유의미한 상관 관계를 보였다(보정된 교차비 1.052, 95% 신뢰구간 1.004-1.104). 청력 역치가 10dB 증가할 때마다 유의미한 후각 기능의 저하가 보였다(보정된 교차비 0.847, 95% 신뢰구간 0.755-0.951). 결론: 본 연구는 만성 부비동염과 청력 역치 상승 간의 유의미한 관련성을 보여주며, 특히 청력이 더 나쁜 귀에서 이러한 연관성이 두드러졌다. 만성 부비동염 환자의 후각 장애와 청력 역치 감소와 상관 관계가 있다. 이 연구 결과는 만성 부비동염의 관리에 있어 청력 평가의 중요성을 강조하며, 이를 통해 해당 환자들의 삶의 질을 향상시킬 수 있을 것이다. 이 연관성의 기저 메커니즘과 임상적 함의에 대한 추가 연구가 필요하다.
