External Radiation Dosimetry to a Caregiver Exposed by an Adult Patient Treated with 177Lu

177Lu has recently been developed as a radiopharmaceutical, but there is no criterionfor 177Lu in the regulation on radiation protection applied to patients administered radioisotopefor medical purposes. The objective of this study is to estimate the external radiation doses ofcaregivers from patients administered 177Lu, and the length of hospitalization required accordingly. Two factors were considered to allow a more realistic assessment of the external radiation dose. First, radioactive change in the body of patients administered 177Lu was calculated using bio-kineticmodeling, which reflected physical radioactive decay and excretion by metabolism. Second, MCNPcomputational simulations were used to calculate dose rate conversion coefficients, which mean therelationship between the effective dose rate received by caregiver and the radioactivity administeredto patient. The external radiation dose of a caregiver calculated was about 3.5 times less than the U.S. NRC method and did not exceed 5 mSv below 600 mCi (22.3 GBq). Therefore, patients administered177Lu less than 600 mCi (22.3 GBq) are not required to be hospitalized for dose constraints of 5 mSv,but patients needed hospitalization when administered in excess of 290 mCi (10.73 GBq) for doseconstraints of 1 mSv.
