Subcortical Shape Analysis in Correlation with Fatigue in Neuromyelitis Optical Spectrum Disorder and Multiple Sclerosis

Neuromyelitis optica spectrum disorder (NMOSD) is a severe inflammatory disease of the central nervous system (CNS) mostly affecting the optic neuritis, myelitis and distinctive brain lesion. On the other hand, multiple sclerosis (MS) is a demyelinating disorder in the CNS with features of dissemination of demyelination lesions in time and space. To the patients with neurological diseases especially in NMOSD and MS, many factors such as depression and poor sleep are related with fatigue. In the viewpoint of central fatigue, especially brain damage, it was reported that the primary fatigue in patients with MS may stem from centrally mediated processes caused by the conduction issues. However, there are few studies of NMOSD patients about primary fatigue in terms of brain damage. More research related to the primary fatigue in the NMOSD patients are needed to prove the mechanisms of NMOSD and can provide precise treatments to patients. We mainly targeted the subcortical regions and investigated the subcortical surface volume images obtained from both NMOSD and MS group. To observe precise volume change location in the subcortical regions, we used the concept of local shape volume to calculate shape diameter in every vertex in the surface. With the volume information of every vertex, we investigated the correlation with the fatigue score and figured out significant subcortical regions related to the fatigue. The usage of partial correlation coefficient between volume information and fatigue score can assert a statistically meaningful relationship between local shape volume and severity of fatigue. Furthermore, correlating the volume information directly with a specific score can be efficient as this method does not require health control group to contrast. This method can also identify sub-networks that capture specific change of fatigues in patients. In this study, we aimed to investigate the relationship between fatigue and the change of the volume of every subcortical region of the NMOSD and MS patients. The results show that in the NMOSD group, the right thalamus is positive correlated with the FACIT-fatigue scale and in the MS group, the caudate, pallidum, putamen region is positive correlated with the FACIT-fatigue scale. This result is in line with the previous clinical findings and out method discovered more precise statistically significant area for each human brain subcortical area. And our finding implies more localized volume change in NMOSD patients than MS patients as only thalamus region volume change is correlated with fatigue score.
