Purpose: The intention for blood donation has been recently decreased, while the demand for transfusion has been nationally increased with increasing aged populations and decreasing birthrates. To overcome this national issue, It may be necessary to elevate blood donation rates in a middle-aged population group who shows relatively lower rates of blood donation to young-aged population groups. Therefore, we aimed to to identify ecological, multi-level factors associated with the intention of blood donation for workers who are in a middle-aged group to develop effective strategies for increasing blood donation rates. Method: The subjects were 211 workers from three corporations inclucing D corporation in Seoul, S corporation in Gyeonggi-do, and L corporation in Daejeon. The data were collected by a survey using self-administered questionnaires from June 29 to July 31, 2019 and were mainly analyzed by the multiple logistic regression. Results: At the intrapersonal level, attitude (odds ration [OR] = 3.28, p = .036) and self-efficacy of blood donation (OR = 1.33, p<.001) were significantly associated with the intention of blood donation. At the interpersonal level, subjective norm (OR = 1.37, p<.001) was significantly associated with the intention. At the organizational level, exposure of blood donation promotion in workplace (OR = 2.62, p=.042) and reward of workpalce (OR = 3.08, p=.005) were significantly associated with the intention. At the community level, exposure of blood donation promotion in community (OR = 3.19, p=.027) was significantly associated with the intention of blood donation. Conclusion: Factors associated with the intention of blood donation were attitude, self-efficacy, subjective norm, exposure of blood donation promotion and rewards in workplace and exposures of blood donation promotion in communities among workers. Government and blood service centers may consider theses factors identified in this study, when developing blood donation interventions and education.
