Background: This study aimed to evaluate the differences in amygdala volume between patients with major depressive disorder (MDD) and healthy controls selected from the Korean population in particular. Methods: Subjects with MDD (n = 2018) were enrolled in this study and were compared to age-, sex-, and education level-matched healthy controls (n = 20). All participants were subjected to T1-weighted structural magnetic resonance imaging (MRI). We used an automated procedure of FreeSurfer to analyze differences in amygdala volume between two groups. Results: The patients with MDD exhibited reduced right amygdala volume (P < 0.05) compared to healthy controls. However, difference in right amygdala volume was not significant after Bonferroni correction of P < 0.025. The left amygdala showed smaller volume, but was statistically insufficient. Limitations: We relied on a relatively small sample size. Results were not persistent after applying Bonferroni correction. Conclusion: Using automated volumetric analyses on MDD patients, we could not demonstrated fundamental structural alteration of the amygdala. Data supported reduced amygdala volume in both sides, but statistically significant only in the right side before correction. Further studies with larger sample would be needed.

배경: 본 연구의 목적은 한국인을 대상으로 주요 우울증 (major depressive disorder, MDD)를 앓고 있는 환자들과 건강한 비교군에서의 편도(amygdala) 용적을 비교하는 것이다. 방법: 주요 우울증 환자군(n = 18)과 나이, 성별, 교육 수준이 일치되는 건강 대조군(n = 20)을 비교하였다. 모든 참가자의 T1강조 자기공명영상을 촬영하여 FreeSurfer의 자동 기법을 이용하여 두 비교군의 해마 용적 차이를 분석하였다. 결과: 주요 우울증 환자군에서 대조군과 비교하여 오른쪽 편도 용적이 유의하게 (P < 0.05) 감소되어있었으나, Bonferroni correction(P < 0.025) 후에는 유의하게 유지되지 않았다. 왼쪽 편도 용적은 주요 우울증 환자군에서 감소되어 있었지만, 통계적으로 유의하지 않았다. 제한: 표본의 수가 상대적으로 작았고, 도출된 결과가 Bonferroni correction 후 유지되지 않았다. 결론: 주요 우울증 환자에서 자동 용적 분석을 이용하여 편도의 구조적 변화를 규명하고자 하였으나, 감소 경향성은 보였지만, 유의한 결과를 얻지 못하였다. 향후 보다 많은 수의 대상으로 한 연구가 필요할 것으로 판단된다.
