Kinematic analysis of upper extremity motion in hemiplegic patients with stroke

Most stroke patients have impairment of upper extremity function. The Difference in the movement trajectory between normal people and hemiplegic patients with stroke should be considered in the treatment of stroke patients to enhance motor recovery of upper extremity function. Three-dimensional (3-D) kinematic motion analysis has the benefit of providing more quantitative and accurate analysis of motion and could be used for training of upper extremity function. The objective of this study was to get the basic kinematic data of the selected activities of daily living (ADL) tasks and compare it between healthy controls and hemiplegic patients with stroke. Seven ADL tasks were selected in reference to commonly used evaluation tools for upper extremity function of stroke patients, and 3-D kinematic motion analysis was performed with the selected ADL tasks. Twenty healthy controls and 20 hemiplegic patients with stroke were enrolled. Six kinematic parameters were analyzed movement time, curve linearity ratio, peak velocity, angular joint motion, inter joint coordination between shoulder and elbow joints, and compensatory movement of trunk motion. The hemiplegia group had a longer movement time and smaller curve linearity ratio. Hemiplegia patients had lower peak velocity and took longer to reach peak velocity at each phase. Compensatory joint motion was prominent in the tasks that required large movement in one joint, and large compensatory trunk motion was prominent in the desk level task. These findings will contribute to the development of a basic model of task oriented training of upper extremity of stroke patients.
