류마티스관절염 환자에서 Anti-Cyclic Citrullinated Peptide Antibodies 와 Rheumatoid Factor 검사의 진단적 유용성

The rheumatoid factor (RF) is the only serological marker in American College of Rheumatology criteria for the diagnosis of rheumatoid arthritis (RA), but its specificity is not satisfactory for the diagosis of RA. The author evaluated the diagnostic performance of a new anti-cyclic citrullinated peptide (CCP) antibodies test by the enzyme-linked immunosorbent assay (ELISA) for the diagnosis of RA. Anti-CCP antibodies were determined in 152 serum samples: 110 from RA patients, 30 from non-RA patients(rheumatic diseases other than RA), and 12 normal individuals. RF by latex fixation test and immunoglobulin (Ig) A and IgM RFs by ELISA were also assayed in each samples, and the results were compared to anti-CCP antibody for sensitivity and specificity by the receiver-operating characteristic curve on optimal cut-off values. The sensitivity of anti-CCP antibody was highest (80%) and the sensitivities of RF latex fixation test, IgA RF and IgM RF were 71.8%, 45.5% and 32.7% respectively. The sensitivity increased (from 80% to 89.1%) especially in the combination of anti-CCP antibody with RF latex fixation test. The specificity of IgA and IgM RFs by ELISA were highest (95.2%) and the specificity of anti-CCP antibody (92.9%) was higher than that of RF latex fixation test (71.4%). Anti-CCP antibody was positive in 58.1% in 31 RF negative RA patients by latex fixation test. The results said that the combination test of anti-CCP antibody with RF latex fixation test might be useful for the diagnosis of RA because of its higher sensitivity and specificity than RF latex fixation test, and has another advantage that is an easy, convenient ELISA method in performance.
