Objective: To compare the compressive strength of the fractured vertebral bodies stabilized by polymethylmethacrylate (PMMA) cement and by new calcium phosphate cements, for use with percutaneous vertebroplasty. Materials and Methods: Seventy-three vertebral bodies (T3-L2, thoracic group: T3-T8, thoracolumbar group: T9-L2) were collected from 7 fresh human cadavers. Compression tests were done before and after vertebroplasty with use of 3 kinds of calcium phosphate cements - CaP1 (calcium phosphate 1, 5 MPa), CaP2 (calcium phosphate 2, 20 MPa), CaH (calcium hydroxyapatite, 50 MPa) -, and PMMA (80 MPa). We selected cements which had the maximal compression force after vertebroplasty similar to that of the intact human vertebral body. Thereafter, we performed the 2nd compression test with selected cements in 18 paired two-vertebrae functional spinal units (FSUs) collected from 3 fresh human cadavers, after cement injection into the lower vertebral body of 12 FSUs, in order to analyze the tendency of inducing compression fracture of the upper vertebral body, compared to the other 6 untreated control FSUs. Results: The maximal compression force of PMMA injected vertebrae was significantly increased after vertebroplasty. The maximal compression force of CaP1 injected vertebrae was significantly decreased after vertebroplasty in thoracic group. The maximal compression forces of CaP2, CaH injected vertebrae were not significantly different from that of untreated human vertebrae. In the 2nd compression test with FSUs, the maximal compression forces of CaP2 or CaH injected FSUs were not significantly different from that of the untreated control FSUs. Conclusion: The CaP2, CaH injected vertebrae showed restoration of the initial maximal compression force of human vertebrae without significant difference. The CaP2, CaH injected functional spinal units showed no tendency of inducing compression fracture of adjacent vertebral body.
